Improve your medication safety and health outcomes
At Manningham General Practice, we can help eligible patients access a Home Medicines Review (HMR).
A Home Medicines Review is a service where an accredited pharmacist visits you at home and reviews your medications, as well as provides recommendations to your GP. The aim is to help ensure your medicines are being used safely and effectively, while reducing the risk of medication-related problems. HMR Referrals Australia states that the review is conducted by an accredited pharmacist, with findings provided to the GP for follow-up care.
What Is a Home Medicines Review?
A Home Medicines Review is a comprehensive assessment of all the medications you take, including prescription medicines, over-the-counter products, vitamins and supplements.
The accredited pharmacist will discuss:
- What medications you take
- How and when you take them
- Any side effects or concerns
- Whether there may be interactions between medicines
- Ways to improve medication management
Following the review, the pharmacist prepares a report for your GP, who will discuss any recommendations with you.
Who may benefit?
A Home Medicines Review may be suitable if you:
- Take multiple medications
- Have recently been discharged from hospital
- Have a chronic or complex medical condition
- Experience medication side effects
- Have difficulty remembering when to take your medicines
- Have concerns about your medication regimen
- Use several different healthcare providers or pharmacies
Older adults and people taking five or more medications are often among those who benefit most from a medication review.
